[ERMCommittee] Draft of Blurb

Thanks to everyone for looking this over.

After chatting with Megan, we're going to use checkboxes (instead of radio
buttons) on the form, so people can choose more than one option of interest.

CARLI-ers, is this ready to be posted? Or right after the new year?
