[ISL-FY24-OERGrant] Sample Effort Reporting form

Hey everyone!

This is a sample of what I have my faculty members (and other individuals on the grant) submit to our Department of Finance and Analysis as "proof" of work on the grant for any future auditing purposes, and then they calculate the numbers that will be reflected in the report that I'll submit to the Illinois State Library.

This is something the Finance Department created for me as the PI to help keep track of effort.

I mentioned this is the CARLI meeting yesterday and it sounded like a bunch of people were interested in looking at it.
